Evolving landscape of primary healthcare, and the need to improve quality of care has increased the importance of having information available to us on a real-time basis; when and where we need it.

Canada Health Infoway and HQIC Collaborate to Empower Patients and Improve Clinical Efficiency

New project will enable patients to access information from their health records and receive valuable services such as requesting prescription renewals and receiving healthcare remotely- all online

July 31st, 2014- Brampton, ON

Patients and clinicians of three Family Health Teams, Dufferin Area, London and Wise Elephant, will soon benefit from using digital health technologies as a result of a $1,000,000 investment made by Canada Health Infoway and Health Quality Innovation Collaborative (HQIC).

‘Our goal is to help Family Health Teams increase patient satisfaction and improve clinician and practice efficiency,’ said Dr. Sanjeev Goel, HQIC’s founder and the project’s sponsor.

The project will integrate HQIC’s Infoway-award winning patient health record portal, miDASH, with the Family Health Teams’ electronic medical record systems. The innovation will enable patients, online or through their mobile devices, to interact securely with their healthcare providers and access information from their health records. By being able to request prescription e-renewals or e-refills electronically, they will also be able to avoid unnecessary office visits.

‘Informed and empowered patients are healthier patients,’ said Dr. Catherine Faulds, Lead Physician at London Family Health Team. ‘Our patients want to renew medications, view their lab results and ask us questions when it is convenient for them. Enabling them to do so online is a priority for us because we will improve service and access while reducing the demands on clinical and administrative staff,’ said Dr. Brian Wilson, Lead Physician at Dufferin Area Family Health Team.

The team here has been working with the Wise Elephant FHT team to improve the rate and quality of patient feedback received. To achieve these goals, we have implemented a method of sending each patient an SMS/text automatically 60 minutes after their appt with one question only. Examples of questions that have been asked are: "How was your experience with the reception staff on your visit to the clinic today?" "How was your experience with the parking to the clinic today?" "Do you have enough time to discuss your problems with your health care provider today?". Once the text is received the patient has an opportunity to respond with "e" "o" or "p" that corresponds to excellent, ok and poor respectively. The response was patients has been remarkable with over 500 patients responding in the past month. The next steps will be to align these questions with the new PCPM framework coming from Health Quality Ontario. See the graph of a sample of the results for the past 1 week.

What is synapse?


Synapse is a collaborative social platform for health care professionals. The platform was created by a non profit “Health Quality Innovation Collaborative”. There is no cost to be join the platform.Synapse offers you the ability to stay connected with your team from almost anywhere (provided there is an internet connection of course) virtually eliminating downtime. Imagine being able to work on documents in real-time with multiple associates separated by hours of travel or planning and coordinating events amongst colleagues, which is often a much more simple task in theory than it is in practicse. There is no other system that allows you to collaborate so effectively and reduce so much wasted time.

Why should I join?
We believe that collaboration, sharing of best practices and innovative uses of technology will provide the tools for us to truly transform healthcare in sustainable manner. Synapse is the platform to launched by us to accomplish this mission.

How secure is it?
Synapse is built on the Yammer platform. It is being used currently in a number of health care settings in the US including the Mayo Clinic. Our Synapse platform meets all the requirements for health care privacy mandated by PHIPA and has had a Privacy Impact Assessment and a Threat Risk Assessment done.

Is it free or is there a cost to join?
There is no cost to join. We would be happy to accept any donations to our non profit organization

Can I go ahead and post or create groups?
Yes, any user can create groups public or private and invite others to join the group. You can post within any group you are a member of.

miDASH was awarded as a winner in the Outcomes Challenge by Canada Health Infoway in 2013 in the Patient health portal category.

The Problem
Primary Health Care Teams in this province are disconnected from their clinical and operational performance. This results in an inability to derive critical feedback which could be used to drive clinician behaviour change, , enhanced organizational use of resources and development of innovative best practices

The Solution
The solution to this problem involves developing an easy to use and accessible, secure system for primary care teams to access their data in real time in order inform their behaviour and help with planning of health care resources.

eRaven provides the ability to automate and monitor the patient referral process from beginning to end in real-time drastically improving office efficiency and patient satisfaction.
Health care providers, patients and their caregivers will be automatically notified when there is a change in the status of the referral, allowing all involved parties to stay abreast of the latest developments.


eRaven contributes to increasing patient satisfaction and at the same time reducing care providers workload through automation.

Current Projects
HQIC team helps power HARP initiative at Wise Elephant FHT
HQIC is partnering with Wise Elephant FHT to utilize the recently released HARP tool jointly developed by HQO and CIHI to identify patients at high risk of admission to hospital for the Brampton HealthLinks initiative. The HARP tool requires data from both the primary care EMR system and the local hospital (William Osler Health System) to identify these patients at high risk of admission. The HQIC team is currently working on merging those 2 data sets and will update you as this important work proceeds.

The HQIC team is off to AFHTO in Ottawa. See us at there and take the Pledge. We think that our tools work really well in the FHT environment. The Synapse portal is configured to the workflow of an FHT and provides analytics to support implementation of QIPs.

Dr. Sanjeev Goel
is a family physician practicing in “downtown” Brampton for over the past 10 years. Born in Montreal, he grew up in Mississauga and obtained his medical degree from University of Toronto in 1995. He completed his family medicine residency in 1997 from St Mary’s Hospital in Montreal before coming to “set up shop” in Brampton. Currently, Dr. Goel is the Lead Physician for the Wise Elephant Family Health Team in Brampton. He also works as a Rehab hospitalist at William Osler Health System, is a Coroner for the Region of Peel and is the Primary Care Lead for the Central West Diabetes RCC.

is a family physician practicing in “downtown” Brampton for over the past 10 years. She obtained her medical degree from University of Toronto in 2000. She then went on to complete her family medicine residency in 2002 from the University of Toronto Hospital before coming to “set up shop” in Brampton. Currently, Dr. Banerjee is Practicing comprehensive family practice with low risk obstetrics at the Wise Elephant Family Health Team in downtown Brampton. She also works as an asistant Clinical Professor, McMaster Family Medicine Program, a Physician Leader, OB Rota, at the William Osler Health System, and a Physician Leader for Domestic Violence Program at Peel Public Health & CFPC Southern Ontario.

Andrew Hussain
Regional CIO and eHealth Lead for the Mississauga Halton Local Health Integration Network, is highly recognized for his work in Information Management and Technology Services. Andrew is well regarded in the industry and has over 17 years of extensive experience in Information Management and Technology Services both in Canada and the US. Andrew’s strategy work was recognized at the 2006 Canadian Information Productivity Awards and was the recipient of the 2005 TELUS Award forbInnovation. Prior to joining the Mississauga Halton LHIN, Andrew was with the Work Place Safety and Insurance Board of Ontario (WSIB) and Trillium Health Centre. His diverse background in both private and public sectors in a variety of business verticals including health care has complemented his role as the Regional CIO for the LHIN. In 2009, Andrew assumed additional responsibility for the Central West Local Health Integration Network. Andrew is the only shared Regional Chief Information Officer and eHealth lead in the Province of Ontario. His portfolio includes both the Mississauga Halton and Central West LHIN.

Dr. John Tracey
graduated in medicine from Trinity College Dublin in 1971. After working in family practice in several rural settings in Canada he settled in Brampton Ontario where he currently works in full time clinical comprehensive family practice as lead physician for North Peel Family Health Team. His life`s work encompasses a comprehensive practice that included emergency medicine, maternal care, hospital care and elder care as the medical director of Peel Manor Home for the Aged Outside of his clinical work he is known for his leadership and passion in advocating for advancement in the delivery of primary care, specifically advocating that every patient should have a family physician. From the mid-nineties to today his medical-political journey has culminated in positions on the boards of both the Ontario Medical Association (current) and the Canadian Medical Association (past 6 year tenure) . He has contributed tirelessly on numerous committees of both boards and continues to advocate for improvements and transformational change in the healthcare systems at a provincial and national level.

Dr. Archna Narula
hBsc, MD, CCFpis a family physician and is completing her masters in public health with a global health focus at the university of Toronto. Dr. narula provides primary care services at : (1) the Shelter health network in hamilton where she provides primary care services to individuals who are homeless or marginally housed and who have complex health and social needs; (2) the Refuge Centre for newcomer health in hamilton where she provides care to new immigrants and refugees; and (3) at the wise Elephant Family health Team in Brampton where she provides full scope primary care and low risk obstetrical care through the the OB Rota group at Brampton Civic hospital. Dr. narula is an Assistant Clinical professor with the Department of Family Medicine at McMaster university where she is involved in the development of the global health curriculum and provides teaching and supervision to residents.

Kristi is a Health Services Professional with over 15 years ofexperience in a variety of health care environments, with particular expertise in program developmentand delivery, and the patient experience. Michelle Karker
has had a 20 year career in the not for profit sector and has spent the last 10 years in health Care leadership. As the Executive Director of the East wellington Family health Team for the past 5 years, she has partnered with the community, the Board of Directors, Ontario government and private developers in order to build two new medical facilities that now ensure thousands of residents have a primary care physician and team caring for them. Michelle has been highly engaged in and has held leadership roles in a variety of local and provincial health care initiatives and associations. As a seasoned leader who embraces strategic change she is excited about how technology can provide not only system efficiencies but can also help improve the health outcomes of our patients. Michelle Rey
is the interim Director, Performance Measurement & Reporting in the Health System Performance branch of Health Quality Ontario (HQO). Michelle has experience in performance measurement, system monitoring, data quality, quality improvement and system accountability. Michelle is the lead for public reporting at HQO including the Quality Monitor Report, a system level analysis of the healthcare system presented to the Minister of Health and tabled in the legislature, and is HQO’s co-lead on the Primary Care Performance Measurement Initiative. Michelle previously held research and administrative roles with MediResource Inc., Hospital for Sick Children and Sigma Aldrich. Michelle has a PhD in Medical Genetics from the University of Toronto, and Master and Bachelor of Science degrees in Biochemistry from Queens University.

Dr. George Southey
is the founder and Executive Lead of Dorval Medical Associates. Dr. Southey has led the growth of the Dorval Medical practice and prompted its emergence as a leading Ontario full-scope, comprehensive family practice through adopting a team-based, patient-centered approach, new technologies, and innovations. Dorval Medical was Ontario’s first Family Health Network. Dr. Southey also serves as a Family Practice Quality Auditor and Physician Review Program Assessor for the College of Physicians and Surgeons of Ontario. Dr. Frank Martino
Frank Martino, MD, Corporate Chief of Family Medicine, William Osler Health System. President, The Ontario College of Family Physicians, Primary Care Lead, Central West LHIN Frank has been practising comprehensive Family Medicine in Brampton for 22 years. He is a graduate of the Family and Emergency Medicine Residency Program at the University of Toronto. He is still active in Obstetrics, inpatient care and the Emergency Department at Brampton Civic Hospital. In 2008, he received the Reg Perkins Award as Ontario Family Physician of the Year. Currently an Associate Clinical Professor at McMaster University, Frank has taught Family Medicine residents and medical students for over 15 years. He was instrumental in bringing a Family Medicine teaching program to Osler through McMaster University in 2007. The program now has 20 residents training at both of Osler’s campuses. In January 2012, Frank was appointed Primary Care Lead for Central West LHIN, mandated to improve primary care engagement and integration into the healthcare system. He is currently serving as Board Chair of the Ontario College of Family Physicians and continues to be active in Medical Practice Guidelines generation and implementation, having presented on this topic at local, provincial and national conferences.Frank serves as President of the Ontario College of Family Physicians (2012/13) and also sits on various committees that seek to improve quality and the delivery of primary care in Ontario.

Dr. Karim Keshavjee
is a Family Physician with a part-time practice in Mississauga. He is the CEO of InfoClin, a company that provides health informatics consulting to a variety of organizations across North America. Karim was the Project-Director for the COMPETE research program at McMaster University and has been a consultant to Canada Health Infoway for their pan-Canadian electronic prescribing project (CeRx) and the inter-operable electronic health record (iEHR) project. Karim is currently the Clinical Data Architect and EMR Consultant for CPCSSN, Canada’s primary care chronic disease surveillance network, which is managed by the College of Family Physicians of Canada. Karim is an Associate Member of the Centre for Evaluation of Medicines at McMaster University and an Adjunct Assistant Professor at the University of Victoria.

Dr. David Daien
BSc(EE), MD, CCFp As a former electrical engineer, an experienced medical informatics consultant, and a practicing full-time family physician, Dr. Daien has steadfastly pursued his interests in the appropriate use of technology to improve the quality of health care. Dr. Daien is a founding board member and vice-chair of the Board of Directors of the Summerville Family health Team (FhT) where he currently practices. As the IT and Quality Lead for this large FhT, he is responsible for the implementation and ongoing operation of a multisite, multi-disciplinary electronic medical record (EMR) with full integration to a large regional hospital. he is also responsible for designing and delivering the Board quality strategy and led the drafting of the FhT’s first Quality Improvement plan (QIp). Dr. Daien is a Lecturer at university of Toronto and assists with resident education in Quality Improvement at the teaching unit located within his FhT. In addition, he is a peer Leader for OntarioMD where he mentors physicians in implementing and optimizing their EMRs and serves on the joint OntarioMD-ehealth Ontario-Canada health Infoway oversight committee for this extensive and successful peer support network. he is a graduate of the OMA/CMA physician leadership development program and a member of the OMA ehealth policy working group. Dr. Daien is also a co-lead of the ConnectingGTA Clinical working Group and provides the clinical voice to the Steering Committee for the ConnectingGTA program – a multi-LhInregional information exchange. Finally, Dr. Daien is co-chairing the Southeast Mississauga health Link.
